Starting May 2026, a PA (Prior authorization) is required for non-billable ICD codes (diagnosis codes).
The Oregon Health Plan will ONLY cover the following diagnoses without a PA:
Low back pain (acute and chronic)
Neck pain and many cervical spine disorders
Thoracic and lumbar spine conditions
Sciatica and some nerve root disorders
Migraine diagnosed by a Doctor
Knee osteoarthritis diagnosed by a Doctor
Scoliosis
Some pregnancy-related conditions, such as nausea/vomiting and breech presentation
Post-stroke depression
Smoking cessation
Contact the office if you need to submit a PA for a condition not listed above.
